Five practice questions, worked

Original questions written to this exam's published syllabus and pattern. Not past papers.

1. In a SOAP progress note, which entry belongs under 'O'?

  1. Suspected traumatic reticuloperitonitis, improving since admission
  2. Rectal temperature 40.2 °C; heart rate 96/min
  3. Repeat haematology tomorrow and start the agreed treatment plan
  4. Owner reports the cow has eaten less since yesterday evening
Answer and working

B. Rectal temperature 40.2 °C; heart rate 96/min

O is objective: measured findings such as temperature and heart rate. The owner's report is subjective (S), the suspected diagnosis and trend are the assessment (A), and tests and treatment are the plan (P).

2. During the night shift, a supervising clinician asks an intern by phone to give an inpatient an injection 'of the usual'. The intern is unsure which drug is meant. What should the intern do?

  1. Give the drug most often used for that condition, because night calls should be kept short
  2. Leave the injection until the morning round without recording the call or telling anyone
  3. Ask the animal attendant which drug was used last time and give the same amount again
  4. Ask the clinician to state the drug, dose and route, and record the instruction before giving it
Answer and working

D. Ask the clinician to state the drug, dose and route, and record the instruction before giving it

An unclear instruction is clarified, not guessed: drug, dose and route are confirmed and recorded. Guessing, relying on an attendant's memory or silently omitting a treatment all endanger the patient and break the rules for supervised practice.

3. What should the final part of a referral letter to another hospital state clearly?

  1. The intern's assessment marks so far in the current internship posting rotation
  2. The referring clinician's personal opinion of the owner's ability to pay for treatment
  3. The specific question the referring clinician wants the receiving clinician to answer
  4. A complete copy of every textbook page used to reach the provisional diagnosis
Answer and working

C. The specific question the referring clinician wants the receiving clinician to answer

A referral letter gives identity, history, findings, tests, treatment with times of last doses, and the specific question. The receiving clinician needs a clear request, not opinions about the owner, copied textbooks or the intern's marks.

4. Which carcass must not be opened in the abattoir or on the farm?

  1. A carcass of an animal with chronic fascioliasis lesions in the liver
  2. A carcass of an animal in which bruising was seen on the shoulder
  3. A carcass of an animal casualty-slaughtered after a leg fracture
  4. A carcass of an animal suspected to have died of anthrax
Answer and working

D. A carcass of an animal suspected to have died of anthrax

Opening an anthrax carcass exposes Bacillus anthracis to air, which lets it form resistant spores that contaminate the site for decades. A peripheral blood smear is taken instead. The other conditions are routinely judged at post-mortem inspection.

5. During a village mass vaccination campaign, why must a fresh needle be used for each animal?

  1. Reusing needles is allowed only for live vaccines, which are self-sterilising
  2. Reusing needles is permitted if the needle is wiped with a dry cotton swab
  3. Reusing needles transmits blood-borne infections such as anaplasmosis and bovine leukosis
  4. Reusing needles changes the vaccine dose because each needle retains half a dose
Answer and working

C. Reusing needles transmits blood-borne infections such as anaplasmosis and bovine leukosis

Needles carry blood between animals and spread agents such as Anaplasma and bovine leukaemia virus. Needles do not hold half a dose, live vaccines do not sterilise needles, and wiping with a swab does not remove blood-borne agents.
