Choose an answer, then open the working. Each explanation says why the right answer is right and why the others are not.
A prescription reads '.5 mg'. Why is this written form prohibited in most institutions?
C. A lost leading zero reads as 5 mg — a tenfold overdose
The naked decimal is banned because if the point is missed or the fax quality is poor, '.5 mg' reads as '5 mg'. The matching rule bans trailing zeros — '5.0 mg' can read as '50 mg'. Both conventions exist because both errors have killed patients, and both are invisible to someone reading quickly.

At which stage do medication errors commonly occur?
A. At any stage from prescribing to monitoring
Errors are grouped by stage — prescribing, transcribing, dispensing, administering and monitoring — and administration is the last chance to intercept the others.

Why does this manual deliberately give no doses?
B. Doses depend on the drug, patient and setting and must come from current sources
A number in a manual would be read as an instruction. Doses are taken from the prescription and current local references.

What is the nurse's role at the administration stage?

Why must the numeracy part of some licensing examinations be passed at a very high mark?
A. A single slip there causes catastrophic harm
A single decimal slip can deliver ten times the intended dose, so near-perfect accuracy is expected.

What method should be used for every calculation?

What makes a double check independent?
D. The second person works it out without being told the first answer
Being told the figure anchors the checker, who then confirms rather than calculates.

What does a do-not-disturb protocol involve?
A. A visible signal and protected time for the round
A tabard or vest and a designated area signal that the nurse should not be interrupted except for emergencies.

Why is hiding an error the worse error?
B. Nobody treats a harm that nobody knows about
Concealment prevents monitoring and treatment of the harm and stops the system learning.

A patient was off the ward when a dose was due. What is recorded?
