1 · End-of-manual check · medium

Leaning on the chest between compressions is harmful because it:

  1. Increases the risk of rib fracture
  2. Slows the compression rate below the target of the guidelines
  3. Prevents full recoil, so the heart refills less
  4. Reduces the rescuer's endurance
Answer and working

C. Prevents full recoil, so the heart refills less

Full recoil allows the heart to refill between compressions, and leaning prevents it, so each subsequent compression ejects less blood. The rescuer feels as though they are working hard while delivering less circulation than shallower compressions with complete release would.

2 · End-of-manual check · medium

Why is blood pressure a poor early indicator of shock in a young, previously fit patient?

  1. Compensation maintains it until it fails abruptly
  2. It is technically difficult to measure accurately
  3. It is affected more by anxiety than by blood loss
  4. It changes only after treatment has begun
Answer and working

A. Compensation maintains it until it fails abruptly

Young patients compensate efficiently and hold a normal blood pressure until the compensation exhausts, at which point it falls suddenly. Rising pulse, cool peripheries, prolonged capillary refill and restlessness appear much earlier, and waiting for hypotension means escalating late in the patients who deteriorate fastest.

3 · Circulation and Chest Compressions · medium

How do chest compressions create blood flow?

  1. By restarting the heart's electrical rhythm directly with each compression
  2. By pushing air into the lungs with each compression
  3. By squeezing the heart and raising pressure in the chest
  4. By warming the blood as it passes through the heart
Answer and working

C. By squeezing the heart and raising pressure in the chest

Compressions generate flow by direct cardiac compression and raised intrathoracic pressure.

4 · Circulation and Chest Compressions · medium

When should an automated external defibrillator be applied?

  1. After ten minutes of compressions
  2. As soon as one is available
  3. Only after rescue breaths are given
  4. Only if a doctor is present
Answer and working

B. As soon as one is available

Early defibrillation of a shockable rhythm is the strongest predictor of survival.

5 · Choking · medium

An adult with severe choking is conscious. What is the first action?

  1. Up to five back blows
  2. Abdominal thrusts until the object comes out
  3. A blind finger sweep of the mouth
  4. Lay them flat and start compressions
Answer and working

A. Up to five back blows

Back blows come first, followed by abdominal thrusts if they fail, alternating.

6 · Bleeding, Shock and Wounds · medium

What should be done with an object embedded in a wound?

  1. Leave it and pad around it
  2. Pull it out quickly
  3. Push it further in
  4. Cover it with a tight bandage
Answer and working

A. Leave it and pad around it

Removing it may cause catastrophic bleeding; it is stabilised in place.

7 · Common Medical Emergencies · medium

A person who has fainted does not recover quickly when laid flat. What is correct?

  1. Treat it as more than a faint and seek help
  2. Sit them up and give them a sugary drink
  3. Leave them to sleep it off
  4. Splash cold water on their face
Answer and working

A. Treat it as more than a faint and seek help

A simple faint resolves fast; failure to recover suggests a more serious cause.

8 · After the Emergency · medium

Why is resuscitation training repeated regularly?

  1. Guidelines change every week
  2. Equipment needs replacing
  3. It is needed for promotion only
  4. Competence decays within months
Answer and working

D. Competence decays within months

Skills decline measurably soon after training in every group studied.

9 · When Help Is Far Away · medium

How can untrained bystanders help?

  1. By being given clear, specific tasks
  2. By being sent away from the scene
  3. By being asked to stay silent
  4. By being told to do compressions without guidance
Answer and working

A. By being given clear, specific tasks

Bystanders can fetch help, a defibrillator or take turns with compressions when directed.

10 · Circulation and Chest Compressions · medium

In adult resuscitation without an advanced airway, what is the ratio of compressions to breaths?

  1. 15 to 2
  2. 5 to 1
  3. 30 to 2
  4. 50 to 5
Answer and working

C. 30 to 2

Adult basic life support uses 30 compressions to 2 breaths; 15 to 2 is used by healthcare rescuers for children in many guidelines.
