Which national standard is regarded as the first environmental management system standard, a precursor of ISO 14001?
B. BS 7750 (United Kingdom, 1992)
BS 7750, published by the British Standards Institution in 1992, was the first EMS standard and influenced both EMAS and ISO 14001. The other codes are unrelated standards.

What did the 2024 amendment to ISO 14001 (and other management system standards) add?
B. A requirement to consider whether climate change is relevant to the organisation's context
The 2024 climate action amendment added to clauses 4.1 and 4.2 that the organisation shall determine whether climate change is a relevant issue and that interested parties can have climate-related requirements. It set no neutrality target, sector ban or change to audits.

In ISO 14001:2015, which term replaced the separate terms 'documents' and 'records'?
C. Documented information
The 2015 revision uses 'documented information' for information an organisation must control and maintain, whether as documents or retained evidence. The other terms are not used in the standard.

An aspects register scores significance = severity × likelihood (each 1–5) with a threshold of 12. Slurry store overflow has severity 5 and likelihood 3; office paper use has severity 2 and likelihood 4. Which statement is correct?
B. Slurry overflow, at 15, is significant; paper, at 8, is not
Slurry: 5 × 3 = 15 ≥ 12, significant; paper: 2 × 4 = 8 < 12, not significant. Adding the scores (8 and 6) uses the wrong formula; swapping the scores or calling both significant misreads the data.
Which data source is most useful for identifying aspects of a food processing line?
C. An input–output balance of materials, energy, water, products, wastes and emissions
Process flow diagrams with input–output balances, site walks, monitoring data, incidents and complaints reveal where the process interacts with the environment. Advertising, unsupported opinion and share prices do not.

A dairy scores aspects with severity × likelihood and treats 12 as the threshold. An effluent discharge subject to a permit limit scores 9. What is good practice?
A. Treat it as significant because it is subject to a legal requirement, and document this rule
Many registers treat any aspect with a legal limit, recent complaint or stakeholder concern as significant regardless of score, because criteria must include compliance obligations. Ignoring, deleting or moving the threshold to suit is poor practice.

Water intensity falls from 4.0 to 3.4 m³ per tonne. What is the percentage improvement?
D. 15 %
Improvement = (4.0 − 3.4)/4.0 = 15 %. 0.6 is the absolute change; 18 % divides by the new value (0.6/3.4); 85 % is the new value as a share of the old.
A plant processing 13000 t at 4.0 m³/t cuts intensity to 3.4 m³/t while throughput rises 20 %. What happens to absolute water use?
C. It rises from 52000 to 53040 m³
New use = 3.4 × (13000 × 1.2) = 3.4 × 15600 = 53040 m³, above 52000: intensity improved 15 % but absolute use rose. 44200 ignores the throughput rise; 52000 assumes no change; 41600 wrongly subtracts the 20 % throughput change instead of adding it.
A food factory generates 240 t of waste, of which 180 t is sent to recycling, composting or digestion. What is its waste diversion rate?
A. 75 %
Diversion = 180/240 = 75 %. 25 % is the share landfilled; 133 % inverts the ratio; 60 % is a subtraction slip (240 − 180 = 60).
Which finding would a certification auditor normally grade as a major nonconformity?
D. No internal audit of the EMS has ever been done
A complete absence or breakdown of a required element, such as internal audit, raises doubt about the system's ability to achieve outcomes and is major. Isolated lapses in an otherwise working control are usually minor.
