1 · Triage · easy

Which group of findings belongs to the 'disability' part of the primary survey?

  1. Capillary refill time, pulse quality and mucous membrane colour
  2. Respiratory rate, breathing effort and lung sounds on auscultation
  3. Stridor or stertor indicating partial obstruction of the airway
  4. Mentation, pupil size and response, and ability to move the limbs
Answer and working

D. Mentation, pupil size and response, and ability to move the limbs

Disability is the neurological assessment: level of consciousness, pupils and motor function. Capillary refill and pulses belong to circulation, rate and lung sounds to breathing, and stridor to airway.

A dog with head trauma has a modified Glasgow Coma Scale score of 8. How is this interpreted?

  1. A low score on a 3-18 scale, indicating a poor prognosis
  2. A high score on a 0-10 scale, indicating near-normal function
  3. A middling score on a 0-18 scale where higher means worse
  4. A motor-only score that ignores the brainstem reflexes
Answer and working

A. A low score on a 3-18 scale, indicating a poor prognosis

The modified Glasgow Coma Scale adds motor activity, brainstem reflexes and level of consciousness, each scored 1-6, for a total of 3-18; lower is worse, and a score of 8 is associated with a poor prognosis. Students often reverse the direction because the Animal Trauma Triage score runs the other way (higher is worse).

3 · Shock · medium

A 30 kg dog in hypovolaemic shock is to receive an isotonic crystalloid bolus of 15 mL/kg, then be reassessed. What volume is given?

  1. 45 mL
  2. 2,700 mL
  3. 450 mL
  4. 675 mL
Answer and working

C. 450 mL

30 kg x 15 mL/kg = 450 mL. 2,700 mL is the old full 'shock dose' of 90 mL/kg (30 x 90), 675 mL is a quarter of that, and 45 mL is a decimal slip. Bolus figures of 10-20 mL/kg are commonly cited — confirm against the product label and your national formulary.

4 · Fluid therapy · easy

Which ECG changes are typical of severe hyperkalaemia?

  1. Tall peaked T waves, absent P waves and bradycardia
  2. U waves, ST-segment depression and tachycardia
  3. A long QT interval, tall P waves and tachycardia
  4. A short PR interval with delta waves and tachycardia
Answer and working

A. Tall peaked T waves, absent P waves and bradycardia

Hyperkalaemia produces peaked T waves, flattening and loss of P waves, QRS widening, bradycardia and finally a sine-wave rhythm. U waves and ST depression are associated with hypokalaemia.

How does calcium gluconate help a cat with life-threatening hyperkalaemia?

  1. It lowers serum potassium within minutes by driving potassium rapidly into cells
  2. It increases renal potassium excretion within a few minutes of injection
  3. It binds potassium in the gut so that it is removed in the faeces
  4. It restores the gap between resting and threshold potentials, protecting the heart
Answer and working

D. It restores the gap between resting and threshold potentials, protecting the heart

Calcium raises the threshold potential and restores myocardial excitability without changing serum potassium; it buys time while insulin-dextrose or bicarbonate shift potassium into cells and fluids or relief of obstruction remove it.

6 · Blood transfusion · medium

Which dog most clearly needs a crossmatch before transfusion?

  1. A DEA 1-negative dog never transfused, receiving DEA 1-negative blood
  2. A dog that received a transfusion two weeks ago and needs another
  3. A healthy registered donor dog attending its routine blood donation
  4. A dog receiving its own blood back by autotransfusion during surgery
Answer and working

B. A dog that received a transfusion two weeks ago and needs another

A previously transfused dog may have formed antibodies after its first transfusion (commonly after about four days), so a crossmatch is required. A never-transfused, type-matched dog is low risk, and a donor or autotransfusion recipient does not receive foreign red cells.

7 · Blood transfusion · easy

Within what time should a unit of blood usually be completed once the transfusion has started?

  1. About 24 hours
  2. About 10 minutes
  3. About 12 hours
  4. About 4 hours
Answer and working

D. About 4 hours

Blood is started slowly while the patient is watched, then given so the unit finishes within about 4 hours, to limit bacterial growth at room temperature. Ten minutes risks volume overload and missed reactions.

8 · Toxicological emergencies · easy

Which form of vitamin K is used to treat anticoagulant rodenticide poisoning?

  1. Vitamin K1 (phytonadione)
  2. Vitamin K3 (menadione)
  3. Vitamin E (alpha-tocopherol)
  4. Protamine sulphate
Answer and working

A. Vitamin K1 (phytonadione)

Vitamin K1 is effective, given for several weeks for second-generation compounds with a prothrombin time check 48-72 hours after the last dose. Vitamin K3 is ineffective; protamine reverses heparin, not vitamin K antagonists.

9 · Trauma · easy

A dog hit by a car breathes rapidly and shallowly with quiet lung sounds dorsally. What is the most likely problem?

  1. Pneumothorax
  2. Laryngeal paralysis
  3. Feline-type asthma
  4. Tracheal collapse
Answer and working

A. Pneumothorax

A rapid, shallow, restrictive pattern with quiet dorsal lung sounds points to air in the pleural space. Laryngeal paralysis and tracheal collapse cause noisy upper-airway breathing; lower airway disease causes expiratory effort and wheezes.

10 · Humane euthanasia · medium

A conscious feral kitten cannot be restrained for intravenous injection. Which route for barbiturate euthanasia is acceptable?

  1. Intraperitoneal injection, accepting a slower onset
  2. Intracardiac injection while the kitten is conscious
  3. Intramuscular potassium chloride without anaesthesia
  4. Oral barbiturate solution squirted into the mouth
Answer and working

A. Intraperitoneal injection, accepting a slower onset

Intraperitoneal barbiturate is acceptable in small animals when IV access is impractical, though onset is slower. Intracardiac injection requires unconsciousness, and potassium chloride requires general anaesthesia.
