1 · Case handling in the Veterinary Clinical Complex · medium

A buffalo is found dead overnight with dark, unclotted blood oozing from the nostrils and anus. The owner asks for a post-mortem at the clinical complex. What is the correct first step?

  1. Collect a peripheral blood smear and stain it for anthrax before any carcass is opened
  2. Open the carcass at once in the post-mortem hall to look for haemorrhagic enteritis
  3. Send the whole carcass by public transport to the nearest diagnostic laboratory
  4. Release the carcass for burial on the farm because sudden death needs no work-up
Answer and working

A. Collect a peripheral blood smear and stain it for anthrax before any carcass is opened

Sudden death with bleeding from orifices and unclotted blood suggests anthrax. Opening the carcass lets the bacilli sporulate and contaminate the environment, so a smear from a peripheral vessel stained with polychrome methylene blue comes first, and the authority is informed if positive.

A puppy with vomiting and haemorrhagic diarrhoea arrives with suspected parvoviral enteritis. Where should it enter the hospital?

  1. Into the general waiting area, provided the owner keeps the puppy on a lap
  2. Into the general medicine ward, because isolation is for zoonotic disease alone
  3. Into the surgical ward, so that it is close to staff who place intravenous catheters
  4. Directly into the isolation unit, by a route that avoids the general waiting area
Answer and working

D. Directly into the isolation unit, by a route that avoids the general waiting area

Contagious cases go straight to isolation without passing through shared areas. Isolation protects other patients, not only people, so it is not limited to zoonoses.

A 40 kg diarrhoeic calf has a base deficit of 15 mmol/L. Using a bicarbonate space factor of 0.6, how much sodium bicarbonate (84 mg per mmol) is needed to correct it?

  1. 30.2 g
  2. 50.4 g
  3. 360 g
  4. 15.1 g
Answer and working

A. 30.2 g

mmol = body weight × base deficit × factor = 40 × 15 × 0.6 = 360 mmol; 360 × 84 mg = 30,240 mg = 30.2 g. 50.4 g uses a factor of 1.0; 360 g confuses millimoles with grams; 15.1 g uses a factor of 0.3. Doses in this question are for the arithmetic only — confirm against the product label and your national formulary.

A frightened dog in the clinic snaps when approached. What should the student do before the full examination?

  1. Hold the dog firmly alone and examine fast before the dog has time to react
  2. Ask the supervisor for safe restraint, a muzzle or sedation before continuing
  3. Skip the examination and prescribe on the history the owner has given
  4. Ask the owner to hold the dog's head while the student examines the mouth
Answer and working

B. Ask the supervisor for safe restraint, a muzzle or sedation before continuing

Safety of the patient, owner and staff comes first; restraint or sedation is arranged by trained staff. Asking the owner to restrain exposes them to injury and liability, and prescribing without examination is not acceptable.

5 · Prescribing · easy

What volume of a 10% solution provides 250 mg?

  1. 25 mL
  2. 0.25 mL
  3. 2.5 mL
  4. 10 mL
Answer and working

C. 2.5 mL

10% = 100 mg/mL; 250 ÷ 100 = 2.5 mL. 25 mL treats 10% as 10 mg/mL, and 0.25 mL as 1,000 mg/mL (10 g in 10 mL).

In the EMA categorisation of antibiotics for veterinary use, what does category B mean?

  1. Avoid: not authorised in veterinary medicine and allowed only in exceptional individual cases
  2. Caution: use when no category D drug would be clinically effective
  3. Prudence: use as first-line treatment wherever an antibiotic is needed
  4. Restrict: use only when susceptibility testing shows category C or D drugs will fail
Answer and working

D. Restrict: use only when susceptibility testing shows category C or D drugs will fail

The categories are A Avoid, B Restrict, C Caution and D Prudence. Category B includes classes critical for human medicine and is reserved for susceptibility-guided use.

7 · Client communication · medium

In the Calgary-Cambridge guide, which two tasks run continuously through the whole consultation?

  1. Providing structure and building the relationship
  2. Taking payment and booking the next appointment
  3. Recording vital signs and updating the problem list
  4. Explaining prognosis and obtaining signed consent
Answer and working

A. Providing structure and building the relationship

Structure and relationship building are the two threads that run alongside the five sequential stages.

8 · Client communication · easy

What does informed consent for surgery require the owner to understand?

  1. Only the procedure and its cost, since risks would alarm the owner needlessly
  2. Only the risks of anaesthesia, since the choice of surgery itself is the clinician's alone
  3. Only the expected discharge date, since owners rarely ask about other matters
  4. Procedure, reason, benefits, risks, alternatives including no treatment, cost, aftercare
Answer and working

D. Procedure, reason, benefits, risks, alternatives including no treatment, cost, aftercare

Consent is informed only if the owner understands what will be done, why, the risks and benefits, the alternatives and the cost.

9 · Rural camps · easy

Most vaccines carried to a rural camp must be kept within which temperature range unless the label states otherwise?

  1. 2–8 °C
  2. −20 to −10 °C
  3. 15–25 °C
  4. 25–30 °C
Answer and working

A. 2–8 °C

Most veterinary vaccines are stored at 2–8 °C; freezing or heat can inactivate them without visible change.

10 · Ambulatory practice · easy

Which field test estimates the somatic cell count of each quarter of a cow's udder at the cow-side?

  1. The withers pinch test
  2. The FAMACHA test
  3. The tuberculin test
  4. The California Mastitis Test
Answer and working

D. The California Mastitis Test

The CMT reagent reacts with DNA from somatic cells, giving a gel whose grade reflects cell count.
