What this subject covers

The surgical principles common to every species: wounds and healing, asepsis, sutures, haemorrhage, shock, inflammation, tumours, hernias, burns and fluid therapy.

In the exams: China clinical paper surgery 30 (its largest clinical component); ICAR AICE-JRF core group 07; ICAR AIEEA-PG 13 (Surgery & Radiology); PRC Surgery 10%; Nepal NLEV 8 (with radiology and anaesthesia); BCSE 22-25 items; EGEL Plus surgery 15

What ICAR AIEEA asks from it

ICAR AIEEA (PG) — Veterinary Science, Surgery & Radiology

Definitions and one-line facts: phases of wound healing, golden period, sterilisation temperatures and indicators, suture absorption mechanisms and USP sizes, inverting versus appositional patterns, types of haemorrhage, shock classification, hernia parts, burn degrees, dehydration percentages and fluid composition.

1 · Wound healing · easy

Which is the correct sequence of the phases of soft-tissue wound healing?

  1. Inflammation, proliferation (repair), maturation (remodelling)
  2. Proliferation (repair), inflammation, maturation (remodelling)
  3. Maturation, inflammation, proliferation
  4. Inflammation, maturation (remodelling), proliferation (repair)
Answer and working

A. Inflammation, proliferation (repair), maturation (remodelling)

Inflammation (with debridement) comes first, then repair with fibroplasia, angiogenesis and epithelialisation, then remodelling of collagen over weeks to years.

2 · Asepsis · easy

The term that means destruction of all microorganisms, including bacterial spores, is:

  1. Sterilisation
  2. Disinfection
  3. Antisepsis
  4. Sanitisation
Answer and working

A. Sterilisation

Only sterilisation kills spores. Disinfection kills most pathogens on objects; antisepsis does the same on living tissue; sanitisation reduces counts to safe levels.

3 · Asepsis · medium

Which instrument has transverse serrations only on the distal half of its jaws?

  1. Kelly haemostatic forceps
  2. Crile haemostatic forceps
  3. Rochester-Carmalt forceps
  4. Allis tissue forceps
Answer and working

A. Kelly haemostatic forceps

Kelly forceps have transverse serrations on the distal half; Crile forceps have them along the whole jaw; Rochester-Carmalt have longitudinal serrations with cross-hatching at the tip; Allis forceps have interlocking teeth.

4 · Suture materials · easy

Which suture size is the largest in diameter?

  1. 2-0
  2. 3-0
  3. 4-0
  4. 6-0
Answer and working

A. 2-0

In the USP scale, sizes below 0 get smaller as the number of zeros rises, so 2-0 is larger than 3-0, 4-0 and 6-0.

5 · Suture materials · easy

A scalpel handle No. 3 accepts which blade?

  1. No. 15
  2. No. 20
  3. No. 22
  4. No. 24
Answer and working

A. No. 15

Handle No. 3 takes blades 10, 11, 12 and 15; handle No. 4 takes the larger blades 20 to 25.

6 · Shock · medium

A persistently rising blood lactate during resuscitation indicates:

  1. Ongoing inadequate tissue perfusion and anaerobic metabolism
  2. Successful restoration of perfusion
  3. Overhydration
  4. Primary respiratory alkalosis from pain-induced hyperventilation
Answer and working

A. Ongoing inadequate tissue perfusion and anaerobic metabolism

Lactate accumulates when cells rely on anaerobic metabolism because of poor oxygen delivery. Falling lactate indicates response; rising lactate indicates failure of resuscitation.

7 · Tumours · medium

Which is the most common skin tumour of horses?

  1. Melanoma
  2. Squamous cell carcinoma
  3. Sarcoid
  4. Lipoma
Answer and working

C. Sarcoid

The equine sarcoid, associated with bovine papillomavirus, is the most common skin tumour of horses. Melanoma is common in older grey horses specifically.

8 · Hernia · medium

Which muscle is commonly transposed to repair a perineal hernia in the dog?

  1. Semitendinosus muscle
  2. Internal obturator
  3. External abdominal oblique
  4. Latissimus dorsi muscle
Answer and working

B. Internal obturator

The internal obturator muscle is elevated from the ischium and transposed dorsally to fill the ventral part of the defect.

9 · Fluid therapy in surgical patients · medium

Subcutaneous fluid therapy in small animals should use:

  1. Isotonic crystalloid only
  2. 5% dextrose
  3. Hypertonic saline
  4. Sodium bicarbonate 8.4%
Answer and working

A. Isotonic crystalloid only

Only isotonic fluids should be given subcutaneously; hypertonic or dextrose solutions can draw fluid into the space or cause sloughing, and absorption is too slow for shock.
