1 · Beta-lactam antibiotics · easy

Why is a Mycoplasma pneumonia not treated with amoxicillin?

  1. Mycoplasma has no cell wall for beta-lactams to act on
  2. Mycoplasma lives only inside neutrophils, beyond the drug's reach
  3. Mycoplasma inactivates amoxicillin with a plasmid beta-lactamase
  4. Amoxicillin cannot cross the alveolar wall into the lung
Answer and working

A. Mycoplasma has no cell wall for beta-lactams to act on

Lacking peptidoglycan, Mycoplasma is intrinsically resistant to all beta-lactams. Macrolides, tetracyclines, pleuromutilins or fluoroquinolones are used instead.

2 · Tetracyclines · easy

Tetracyclines inhibit bacterial protein synthesis by

  1. blocking aminoacyl-tRNA binding at the 30S acceptor site
  2. inhibiting peptidyl transferase on the 50S subunit
  3. blocking the peptide exit tunnel of the 50S subunit
  4. causing misreading of mRNA after binding the 30S subunit
Answer and working

A. blocking aminoacyl-tRNA binding at the 30S acceptor site

Tetracyclines stop aminoacyl-tRNA reaching the A site of the 30S subunit and are bacteriostatic. mRNA misreading is the aminoglycoside mechanism; the exit tunnel is the macrolide site; peptidyl transferase is the amphenicol and pleuromutilin site.

3 · Fluoroquinolones · easy

In Gram-negative bacteria the primary target of fluoroquinolones is

  1. RNA polymerase beta subunit
  2. Penicillin-binding protein 2a
  3. DNA gyrase, a type II topoisomerase
  4. Dihydropteroate synthase
Answer and working

C. DNA gyrase, a type II topoisomerase

Fluoroquinolones inhibit DNA gyrase, mainly in Gram-negatives, and topoisomerase IV, mainly in Gram-positives.

4 · Fluoroquinolones · easy

Enrofloxacin is partly metabolised in most species to which active compound?

  1. Marbofloxacin
  2. Nalidixic acid
  3. Ciprofloxacin
  4. Norfloxacin
Answer and working

C. Ciprofloxacin

De-ethylation of enrofloxacin produces ciprofloxacin, which adds to antibacterial activity.

5 · Antifungal drugs · easy

Azole antifungals act by inhibiting

  1. beta-1,3-glucan synthase in the fungal cell wall
  2. fungal microtubule assembly during cell division
  3. squalene epoxidase at the early step of ergosterol synthesis
  4. lanosterol 14-alpha-demethylase in ergosterol synthesis
Answer and working

D. lanosterol 14-alpha-demethylase in ergosterol synthesis

Azoles block the cytochrome P450 enzyme lanosterol 14-alpha-demethylase. Squalene epoxidase is the terbinafine target, glucan synthase the echinocandin target and microtubules the griseofulvin target.

6 · Antiprotozoal drugs · easy

Several horses die with myocardial necrosis after being fed a cattle ration. Which feed additive is the most likely cause?

  1. Decoquinate
  2. Amprolium
  3. Monensin
  4. Toltrazuril
Answer and working

C. Monensin

Ionophores such as monensin are extremely toxic to horses, causing cardiac and skeletal muscle necrosis.

7 · Anthelmintics · medium

A 400 kg steer is to receive injectable 1% ivermectin at 0.2 mg/kg subcutaneously (a regimen used here for the arithmetic — confirm against the product label and your national formulary). What volume is required?

  1. 0.8 mL
  2. 80 mL
  3. 8 mL
  4. 20 mL
Answer and working

C. 8 mL

Dose = 0.2 mg/kg x 400 kg = 80 mg. A 1% solution contains 10 mg/mL, so 80 / 10 = 8 mL. 80 mL confuses mg with mL; 20 mL treats the solution as 0.4%; 0.8 mL treats 1% as 100 mg/mL.

8 · Mechanisms and problems of drug resistance · easy

Transfer of a resistance plasmid from one bacterium to another through direct cell-to-cell contact is called

  1. transformation
  2. conjugation
  3. transposition
  4. transduction
Answer and working

B. conjugation

Conjugation transfers plasmids through direct contact. Transduction uses bacteriophages; transformation is uptake of free DNA; transposition is movement of a transposon within or between DNA molecules in one cell.
